Weaning newborn infants from mechanical ventilation

چکیده
Invasive mechanical ventilation is a life-saving procedure which is largely used in neonatal intensive care units, particularly in very premature newborn infants. However, this essential treatment may increase mortality and cause substantial morbidity, including lung or airway injuries, unplanned extubations, adverse hemodynamic effects, analgosedative dependency and severe infectious complications, such as ventilator-associated pneumonia. Therefore, limiting the duration of airway intubation and mechanical ventilator support is crucial for the neonatologist, who should aim to a shorter process of discontinuing mechanical ventilation as well as an earlier appreciation of readiness for spontaneous breathing trials. Unfortunately, there is scarce information about the best ways to perform an effective weaning process in infants undergoing mechanical ventilation, thus in most cases the weaning course is still based upon the individual judgment of the attending clinician. Nonetheless, some evidence indicate that volume targeted ventilation modes are more effective in reducing the duration of mechanical ventilation than traditional pressure limited ventilation modes, particularly in very preterm babies. Weaning and extubation directly from high frequency ventilation could be another option, even though its effectiveness, when compared to switching and subsequent weaning and extubating from conventional ventilation, is yet to be adequately investigated. Some data suggest the use of weaning protocols could reduce the weaning time and duration of mechanical ventilation, but better designed prospective studies are still needed to confirm these preliminary observations. Finally, the implementation of short spontaneous breathing tests in preterm infants has been shown to be beneficial in some centres, favoring an earlier Review Proceedings of the 9 International Workshop on Neonatology · Cagliari (Italy) · October 23-26, 2013 · Learned lessons, changing practice and cutting-edge research Proceedings

Oxygen cost of breathing and weaning process in newborn infants.
Newborn infants may have a high oxygen cost of breathing (OCB) at the time of being weaned from mechanical ventilation. We hypothesized that this increase in oxygen consumption (V'O2) could be reduced by using certain weaning ventilatory modes. We designed a study to assess V'O2 during three weaning ventilatory modes: patient triggered ventilation, synchronous intermittent mandatory ventilation...

BACKGROUND Mechanical ventilation is a life-saving intervention for critically ill newborn infants with respiratory failure admitted to a neonatal intensive care unit (NICU). Ventilating newborn infants can be challenging due to small tidal volumes, high breathing frequencies, and the use of uncuffed endotracheal tubes. Mechanical ventilation has several short-term, as well as long-term complic...

Background and Objective: The results of previous studies show the neonate position is effective in improving oxygenation . Therefore, this study aimed to compare the prone and supine positions on oxygen saturation in preterm infants who were weaned from mechanical ventilation. Materials and Methods: This clinical trial carried out on 40 preterm infants who were weaned from mechan...
متن کاملComparison of the effect of pressure support ventilation and volume assured pressure support ventilation on weaning patients off mechanical ventilation after cardiac surgery
Background: Weaning from mechanical ventilation in the intensive care unit is an important step in the treatment process. More patients in less than 6 hours after cardiac surgery can be separated from mechanical ventilation and extubated. However, 20-40% of patients after cardiac surgery, due to dysfunctional ventilator weaning response (DVWR), still remain under mechanical ventilation. Therefo...

Background and purpose: Respiratory disorders and need for mechanical ventilation are common complications in preterm infants. Âttention has been drawn to premature weaning from ventilator and also removal of endotracheal tube with an approach towards reduction of risks. This study has been carried out to compare the outcomes of two methods of NÏMV and NÇPÂP in infants afflicted with respirator...
